Lifeline Supports Safe Patient Discharge July 12th, 2009

What is ADPCO?
The mandate of A.D.P.C.O. in conjunction with The Canadian Association of Discharge Planning And Continuity of Care (C.A.D.P.A.C.C.) includes the development of guidelines and standards for practice and the establishment of educational programs for its members. "Guidelines and Standards For Discharge Planning Coordinators", developed for the Canadian Council on Health Services Accreditation, drives the practice across Canada.

Objectives of ADPCO:

  1. To provide a structure that contributes to the studying, planning and employing of methods that improvedischarge planning services and contribute to improved standards of care.
  2. To plan educational programs, seminars, workshops and research programs of benefit and interest to members and their colleagues.
  3. To foster cohesiveness between health care professionals who provide discharge planning services.
  4. To identify those factors which impede appropriate use of hospital and community services and recommend necessary changes the appropriate bodies.
  5. To develop and foster partnerships with other organizations having similar objectives to promote positive change.

What is CADPACC?
The Canadian Association of Discharge Planning & Continuity of Care (CADPACC) was formed in 1992 under the sponsorship of the Association of Discharge Planning Coordinators of Ontario (ADPCO) with executive members from across Canada. The impetus for its formation was to provide discharge planners with standards of practice (Discharge Planning Standards developed by the ADPCO) which also meet the Standards of the Canadian council on Health Services Accreditation.

It is an association of health care professionals of varied backgrounds who perform the discharge planning function in hospitals.

Membership is also available to professionals from related organizations such as long term care facilities; home care; community agencies and private sector health care services.

Objectives of CADPACC:

  1. To provide a structure that contributes to the study, planning and employment of methods that improve discharge planning services.
  2. To plan educational programs, workshops and research related to discharge planning.
  3. To foster collaboration between health care professionals who provide discharge planning services.
  4. To identify factors which impede appropriate utilization of hospital and community services and make recommendations for solutions to those barriers.
  5. To develop and foster partnerships with other organizations having similar objectives to promote positive change.

ADPCO Membership

Regular Member
A regular membership is available to staff of Ontario Hospital Association member hospitals who carry out the discharge planning function. A regular member has voting privileges.

Associate Member
An associate membership is available to allied health professionals and to interested community agency personnel. An associate member of the association has no voting privileges.


Applying
An application for membership must be made to the Membership Committee each year along with the payment of the designated fee. Failure to do so will cancel membership
